Background

The field of emergency medicine is growing in urban Nepal.  Large urban hospitals have triage systems and dedicated emergency physicians and nurses. Pre-hospital care has begun in the valley, and attention is paid to disaster preparedness.

While the acute care needs of rural Nepal are best met by MDGPs, there is a desire to have a group of urban physicians with the competence to evolve the discipline of emergency medicine, and emergency medicine systems in Nepal.

Education

The purpose of the EM Fellowship is to take experienced GPs who currently practice emergency medicine and supplement their EM knowledge, skills and attitudes so that they can become teaching faculty for future MD-EM or EM Fellowship programs.

They will be compassionate, competent and patient-centered emergency doctors, able to provide all age groups with comprehensive and effective management of acute health problems.  To improve the health of Nepali people, especially the poor and underserved in rural areas, graduates of the EM fellowship will be leaders in teaching acute care to PAHS students and MDGP residents. 

System Development

EM Fellows are expected to be leaders in pre-hospital care, disaster preparedness, triage and emergency medicine education at all levels, in urban centers.

EM FELLOWSHIP GOAL

Desired Outcome

To produce competent Emergency Medicine specialists who will become clinical and educational leaders able to manage the emergency services systems in Nepal.  The fellowship is a faculty development course that will take advantage of the experience of GPs who provide emergency care.  The fellowship will provide additional education to ensure that they achieve the competence to begin practice as teaching faculty in emergency medicine, and as emergency system managers.

PAHS Core Competencies

They represent the areas in which every physician should be competent.  Because competence is the habitual performance of key behaviors, assessment of competence is done in part by looking for the key, or sentinel habits in the workplace.  Sentinel habits are the essence of a domain of competence. The  competencies and sentinel habits are as below:

Length of Training: The EM Fellowship will be 18 months. 

The program will consist of block rotations, horizontal rotations, and the academic curriculum.  Fellows will be allowed leaves as per PAHS rules and regulations.  

The fellowship has the following elements:

1. Rotation of clinical placements

2. Structured learning activities – weekly classes (full morning of protected time),  monthly core content readings and quiz, journal club, skill sessions, presentations, scholarly project, research article etc

3. Assessment activities

Clinical Rotations

Block rotations: Rotations will be in following blocks

  1. Emergency Medicine  
  2. ICU      
  3. PICU     
  4. Anesthesia    
  5. Trauma and Orthopedics   
  6. ED Systems and Education

Horizontal rotations:  these clinical experiences will occur throughout the fellowship.

  1. Pre-hospital Care / EMS
  2. Disaster Medicine

Rotation-Specific Learning Objectives

Rotation-Specific Learning Objectives are simply a way to reframe the Competencies and Learning Objectives specific to a rotation.  There will be rotation specific learning objectives for each rotation to guide the training.

ADMISSIONS

There will be three types of seats:

A. Sponsored candidates from PAHS

B. Other Institutional sponsored candidates

C. Free candidates

 

The number of seats in the above mentioned categories will be decided by Executive Council in consultation with Program coordinator

Minimum Eligibility criteria for all candidates

1. The candidate must be Nepali Citizen

2. The candidates must have Post graduate MDGP degree recognized by Nepal Medical Council (NMC).

3. The candidate must be registered in Nepal medical council.

4. The candidates must have one year working experience in emergency after completion of MDGP.

Criteria for  Sponsored Candidates from PAHS

1. The candidate must be currently working in PAHS.   

2. He/she must have worked for two years in Patan Hospital after completion of MDGP.

3. The candidate must provide a  letter of approval from Hospital Director .

4. The candidate must sign a bond agreeing to work for number of years as per the regulations of PAHS.

 Criteria for Institutional sponsored candidates

1. The candidate must be currently working in the Institute which has agreed to sponsor the candidate.

2. He/she must provide a letter of sponsorship from the Sponsoring Institute.

Preference for the seats will be in the following order:

1. PAHS sponsored 

2. Institutional ( other than PAHS) sponsored

3. Free candidates

Selection Process

The short listed candidates will be called for an Interview which will be conducted in a form of structured  panel interview.  The best candidates will be selected on merit basis.

LEARNER ASSESSMENT

The system focuses on the development of competence, and will use formative and continuous assessment with a major emphasis on documented feedback and reflective learning to continuously provide the candidate the opportunity to improve the performance and develop the required competencies.  

If there is serious deficiency in the required competencies, the Program Coordinator, in consultation with other Faculties, can extend the required period.

Award of Fellowship 

The successful candidates will be awarded the certificate of Fellowship In Emergency Medicine.

PAHS will not pay any salary or provide any other benefit to Sponsored candidate from other Institution or free candidates.

How to apply

Candidates can obtain forms from Dean's Office on submission bank voucher of Rs 2500 drawn upon PAHS account in Laxmi Bank Ltd., Lagankhel, Lalitpur. Forms should be duly filled and returned to the office by 4 pm September 9, 2013. The date and time of Interview will be notified at a later date.
