Nepal Health Professionals Council (NHPC), established under the Nepal Health Professionals Council Act, 2053, conducts licensing examinations for various health professionals (except doctors, nurses, and pharmacy officials).
After passing the license exam, also known as the name registration exam, the applicant should submit the following documents to get the license certificate.

- One copy of the photo taken while filling the online form.
- Original copy of the bank voucher for the exam fee (check above in the table) submitted to the banks where the council has an account and original copy of the name registration fee voucher submitted according to the level for name registration.
- Photocopy and original copy of Nepali citizenship certificate.
- Photocopies and originals of SLC marksheet, character certificate and provisional/original certificates.
- Photocopies and originals of certificate level marksheet, character certificate and provisional/original certificates and practical exercises.
- Photocopies and originals of undergraduate level (bachelor level) marksheet, character certificate and provisional/original certificates and practical exercises.
- Photocopies and originals of postgraduate level (masters level) marksheet, character certificate and provisional/original certificates and practical exercises.
- Photocopy and original of equivalence certificate available from Tribhuvan University/CTEVT/Curriculum Development Center for students who have studied outside Nepal.
- Photocopy and original of the study consent letter of the council/Medical Education Commission taken while going to study outside Nepal.
- Photocopy and original of the admission card given for the name registration examination conducted by the council.
- For students of certificate level, undergraduate level and postgraduate level who have studied in Nepal, write "The photocopy is correct as per the original" behind the photocopies and sign and submit the photocopies in the record file and the original certificates in a Clear Bag.
- For students who have studied outside Nepal, notarize all the photocopies and submit the notarized photocopies in the record file and the original certificates in a Clear Bag.
- It is informed that the name registration certificate will be made available on the day after the day the file is submitted to the council.
